In order to schedule a new patient appointment we need the following items faxed to our office:
Chart notes pertaining to your spine condition;
Current CT or MRI reports;
Your name along with a current phone number where we can reach you;
Copy of your insurance card(s) - or other insurance information if your spine problem is due to a
worker's compensation claim or a motor vehicle accident.
If you do not have recent notes or current imaging reports, please contact your Primary Care Physician for
medical treatment for your back/neck; he or she can refer you to our office if appropriate.
Our staff will review the information to determine if your medical needs can be treated by our services.
If it appears other treatment will better meet your needs - we will recommend a local office that may be
able to treat you. Our review process is designed to get you into the most appropriate facility in the least
amount of time.
Our review process takes 5-7 days. We will contact you after the review is complete.
